Minors must be accompanied by a parent or legal guardian at every appointment. Treatment of a minor without written parental consent is considered assault. 


We do understand that this is not always possible and sometimes a grandparent or other adult may need to bring the child, or that teenagers may be able to drive, and can come alone. 

For these instances we have a Consent to Treat a Minor form for the parent or legal guardian to fill out and sign.


The parent or legal guardian MUST be at the first initial appointment and any appointment that requires legal consent for a procedure.
