Before your first visit, we make sure to obtain important background information, like your medical history, medications, etc. This allows your time spent at the office to be focused specifically on your visit, not paperwork.
At the time you schedule your appointment, you will be given the option to register for your visit over the phone during a scheduled time with our patient coordinator, or to utilize the Registration Form below. This must be completed at least 48 business hours before your visit in order to prevent your appointment from being rescheduled.
Insurance and Payment Information
You are responsible for co-payments or charges that are not covered by your insurance. If you have questions regarding billing or which insurance plans we accept, please call our office.
For commercial insurance patients: if your deductible has not been met, we will require a $100 (new patient) and $50 (established patient) deposit to be applied to your deductible at the time of your appointment.
Please arrive early so that we can complete your initial paperwork. Also, please bring the following:
